Eligibility: Open to legal residents of the U.S. who are 18 years or older. Employees of UVBioTek, LLC and their immediate family members are not eligible.
Entry: No purchase necessary. Complete and submit the entry form to enter. Limit one entry per person.
Prize: One (1) Poly Go device (approximate retail value: $300). Prize is non-transferable and cannot be exchanged for cash or other items.
Winner Selection: The winner will be chosen at random from all eligible entries and notified via email.
Claiming the Prize: The winner must respond within 30 days of notification to claim the prize. If the winner does not respond, an alternate winner may be selected.
Publicity: By entering, participants agree to allow UVBioTek, LLC to use their name and likeness for promotional purposes without additional compensation, unless prohibited by law.
Liability Release: UVBioTek, LLC is not responsible for lost, late, or misdirected entries, technical errors, or any issues that may affect participation.
Modification/Cancellation: UVBioTek, LLC reserves the right to modify, suspend, or cancel the contest if necessary.
Agreement to Terms: By entering, participants agree to these terms and all decisions made by UVBioTek, LLC, which are final and binding.
